Maynard- James Dale Cook 54, Passed away Friday December 23, 2016 at Five Rivers Medical Center.

James was born January 6, 1962 in Galesburg, IL. to the late Ernest and Mary Kay Cook.

He is survived by his wife of 32 years Brenda Hormann Cook : Children James Cook , Jamie Cook-Franklin, Roni Quinn, Brandon (Devalyn) Cook, Shane (Steffonie) Cook, Misty (Denis) Jones, Ricky Cook, Tommy (Carlee) Shaffer. Grandchildren Ray-Ann, Destiny, Tristian, Tae'ana, Jerzie, Jacob, Olivia, Riley, Bryson, Kaleb, Jacob, Addyson, Kayson, Brayden, Kayden, Kaitlyn, and his siblings, Michael (Ann) Cook, Kim Davis, Sheri (Darren) Baltz.

He is preceded in death by his parents, Ernest and Mary Kay Cook.

Visitation will be Tuesday, December 27, 2016 from 1-2 p.m. with a celebration of life immediately following at McNabb Funeral Home Chapel. Burial will be at Siloam Cemetery.

Pallbearers: Darren Blackman, Ryan Conyers, Josh Miller, Chad Hutchison, Chris Bussell, Cory Kee, and Jay Davis.
